Please print this off if you have a printer and send the slip from the booking form with a cheque for the cost of the event and a stamped addressed envelope for your acknowledgement to the address on the form. If you do not have a printer please write your contact details etc. as the booking form on a piece of paper along with a cheque and stamped addressed envelope for your acknowledgement.
